2010 Call for Awards
ONTARIO BRANCH CSHP AWARDS PROGRAM:
The Awards Program of Ontario Branch, CSHP is designed to recognize excellence in Hospital Pharmacy practice, show appreciation to deserving members of the Branch, and further the mission of the Canadian Society of Hospital Pharmacists.
Applications are judged solely on the data submitted. The Awards Committeewill not conduct any investigation on behalf of the candidates.
Applicants who were previously not selected are encouraged to resubmit. The Awards Committee must receive completed applications no later than July 9, 2010. All awards will be presented in conjunction with the Annual General Meeting of Ontario Branch, CSHP on Saturday, November 13, 2010.
INSTRUCTIONS FOR SUBMISSION:
1. Three types of awards are available: 1) awards for projects/articles; 2) personal recognition awards; and 3) grants.
Match your project, article or nominated colleague to the Terms of Reference provided for each award.
2. An Ontario Branch member may submit for as many awards as he/she has qualifying projects. An individual project/article must be submitted for only one award (exception for residency award submissions).
3. Submit the stated Submission Requirements to Susan Korporal, Administrator, Ontario Branch CSHP, by e-mail at skorporal@cshp.ca.
4. Submit the Application Form, with signatures, to Susan Korporal by email. The Application Form is located on the last page of the Award Descriptions document.
5. If you are unable to submit a signed copy of the application via email, you may alternatively submit the Application Form, with signatures, to Susan Korporal by fax at 613-736-5660, or by mail to the address below.
